ESSENTIAL JOB DUTIES:

• Personal care duties include but are not limited to bathing, grooming, feeding, scheduling appointments, turning and positioning.
• Assist the patient with transfer, ambulating, prescribed exercise as instructed by the Home Care Nurse.
• Observe and report specific vital signs and symptoms as instructed by Home Care.
• When instructed by the Home Care Nurse, record weight, check client's pulse, temperature, respiration rate, measure food and fluid intake and output.
• Prepare modified diet as ordered by the home care nurse.
• Record keeping such as maintaining records on client, services received, current conditions and progress.
• Maintaining a safe, secure and healthy patient environment, by following the asepsis standards and procedures.
• Maintaining client and family confidentiality.

QUALIFICATIONS:

• Have completed a training program approved by the New York State Department of Health as a Home Health Aide or be enrolled as a trainee aide in New York State approved training program under the supervision of qualified RN.
• Be a minimum of 18 years of age.
• The ability to read and write in English.
• Have a physical examination satisfying the New York State Department of Health requirements for Home Health Care Workers.
• Submit annual mandatory compliance documents such as annual physical, QuantiFERON or PPD, a copy of x-ray report (if positive PPD) and annual TB questionnaire.
• Complete 12 hours of in-service annually.

• 4-hour shift
• 8-hour shift
• 10-hour shift
• 12-hour shift
• Create a schedule based upon your availability.

LICENSE/CERTIFICATION:

• PCA or HHA Certification.
